Applications, Participation Forms, and Order Forms

In an effort to assist our members and friends, this page includes applications, sign-up sheets for events, and order forms for fundraisers. We will do our best to keep this list current and post forms as they become available. Please feel free to download and print any of the forms listed below, fill them out, and return them to the address specified on the form. If none is supplied, please return the form to the Temple Office, 75 East James St., Lancaster, PA 17602. If you are looking for a specific event not listed, please contact the Temple Office for information.

**Instructions for forms which allow electronic fill-in: Click on desired form. Select Open. Fill in your information. Save the completed document to your hard drive. Send the file as an e-mail attachment to the address specified on the form or to office@shaarai.org.
